This error could occur unintentionally if the same user that is running the plan on a server is logging onto the same server via a remote desktop session, the plan then starts, and then the user logs off the remote session, as distinct from disconnecting from the session. The system involved in this case is (server2012) and the account is (domainA\Administrator). Please ensure this is not taking place.
